Medical Record Indexing
Indexing that rebuilds the record set
InQuery reads every page of a production, finds where each document begins and ends, collapses the duplicates, and returns a sorted, tagged, filterable record set.
Medical records rarely arrive as records. They arrive as one flattened PDF with no boundaries, the same visit produced by four custodians, and documents that start halfway down a page. We rebuild the documents inside the production and clear the noise, so your reviewers open an organized file instead of paginating one.
What our record indexing includes
- Segment flattened PDFs into individual documents, including boundaries that fall mid-page
- Tag every document with provider, date, facility, and record type
- Collapse duplicates, including partial, re-faxed, and re-scanned copies
- Sort and filter the full production by provider, date, or record type
- Produce page counts you can attest to with confidence
- Flag the noise: blank pages, fax covers, and filler that hide the record
